Two children with perforated choledochal cyst have been
presented. In cases with bile peritonitis, existence of perforation of the
cyst should be considered in the diagnosis. The cases characterize the two
modes of symptoms as in spontaneous bile peritonitis: patients with the
acute form usually complain of vomiting, abdominal pains and distension
several days before admission, while in the chronic form, jaundice and acholic
stool may develop in illnesses with a history of ten days or more. Treatment
of this condition consists of external drainage through the site of perforation
of the cyst, with elective excision of the cyst and hepatico-enterostomy
later.
