病理組織診断、細胞診断学的な研究

研究内容

病理診断科・病理部は臨床面で直接診療に貢献できる多数の症例解析や新しい病理診断法の確立を目指して、臨床的研究にあたっています。

症例解析に関する研究では、日常の病理組織診断や細胞診断での診断困難な症例、良悪性の判定が困難な症例、稀有症例を対象に、組織や細胞所見の解析に加えて、免疫組織化学の活用や細胞増殖因子および癌抑制遺伝子について多数の症例を解析することにより正確で客観的な病理診断を目指す研究を行っています。また核形態計測などを用いて客観的な因子を検討し、予後の推定や治療方針への貢献について研究しています。特定の臓器に関しては、肺疾患、消化器疾患、腎疾患などを中心に病理組織診断や細胞診断学的な研究を行っています。

新しい病理診断法の確立に関する研究では、微小な検体で診断可能な細胞診断を利用して、ベッドサイドで施行可能な新たなる診断法の確立を目指しています。特に呼吸器疾患におけるultra-fast PAP染色を用いたベッドサイド細胞診は、香川大学医学部附属病院で確立された研究成果であるが、迅速診断、EUS-FNA時の診断にも応用し、診断精度の向上に努めたいと考えています。

このように病理診断科・病理部における研究は日常の業務に直結した臨床研究であり、日本の医療に不足している病理医の育成とともに、大学附属病院病理診断科・病理部の重要な課題と言えます。この研究により日常業務での病理診断がより迅速で正確にできることが将来の夢です。尚、現在は全身臓器における病理形態学の豊富な知識を生かして、各講座との共同研究も重視し行っています。

現在まで、病理診断科・病理部内で細胞学的な研究を行っている者や、病理部外の教室と共同研究に参加している者がいますが、良好な関係であるため研究・教育が主体の病理学講座での基礎的な研究分野への参加も可能です。なお、細胞検査士の方が社会人枠を利用して医学博士取得も可能です。

Research Activities

Outline:　Our department engages in clinical research, especially case studies which? contribute directly to clinical medical treatment and new methods of pathological diagnosis.
The objective of our clinical case studies is to make the correct diagnosis of difficult cases in routine pathological diagnosis, difficult cases regarding differentiating between benign and malignant tumors, and very rare diseases. These cases are, for example, lung disease, digestive disease, and kidney disease. We analyze many such cases by histological, cytological, immunochemical, electron microscopical, and immunofluorescence studies. Thus, we aim at rapid and objective pathological diagnosis. Moreover, by morphometric measurement, we seek to examine and predict the tumor biological behavior.???????????
Research on new methods of pathological diagnosis include bed-side cytology with the possibility of diagnosis even with only a few materials obtained from the patient. In particular, the combination of diagnostic accuracy and safety of flexible bronchoscopy with ultrafast Papanicolaou stain in pulmonary nodules has been proven in our Kagawa University Hospital. We plan to apply new study methods for renal disease, effusions in body cavities, and quick frozen section diagnosis. And we are very commited to improving our diagnostic accuracy.
Thus, the main research in the Department of Diagnostic Pathology is clinical research directly related to routine pathological diagnostic work. A key issue is the training of anatomical pathologists, who are lacking in Japan. Our final goal is rapid and objective pathological diagnosis on a day-to-day basis.??

Department Activities

Our staff attend annual meetings of the Japanese Society of Pathology and Clinical Cytology. We often publish papers in histopathological and cytopathological journals. Clinicopathological joint conferences with other clinical departments are held about 2 or 3 times a week.???????

Research Training and Available Techniques

We use methods and techniques employing light and electron microscopy, immunohistochemical staining, and immunofluorescence. Flow cytometry and morphometry are also available.
